If you have received a bill, you have the option to pay online, by phone, or by mail:

ONLINE: Please visit your account portal in StrataEMR.

PHONE: ​You can call and provide your billing information. If your card is on file with ProFormance and you would like to use it to pay your balance, please call or email to let us know. We will bill it for you and send you the receipt.

MAIL: Please remit payments to 216 Bel Air Rd. SE Huntsville, AL 35802.

ProFormance tries to be as eco-friendly as possibly by providing electronic statements. If you have not received a statement/invoice and would like one, please email us.

Debts without payment over 60 days with be assessed a $40 fee per 30 days afterward. If no attempt to pay the debt is made, the debt will be turned over to collections.
